Unmanned Mission Command, Pt. 1

The following two-part series discusses the command and control of future autonomous systems. Part 1 describes how we have arrived at the current tendency towards detailed control. Part 2 proposes how to refocus on mission command.

Introduction

In recent years, the U.S. Navy’s unmanned vehicles have achieved a number of game-changing “firsts.” The X-47B Unmanned Combat Air System (UCAS) executed the first carrier launch and recovery in 2013, first combined manned/unmanned carrier operations in 2014, and first aerial refueling in 2015.1 In 2014, the Office of Naval Research demonstrated the first swarm capability for Unmanned Surface Vehicles (USV).2 In 2015, the NORTH DAKOTA performed the first launch and recovery of an Unmanned Underwater Vehicle (UUV) from a submarine during an operational mission.3 While these successes may represent the vanguard of a revolution in military technology, the larger revolution in military affairs will only be possible with the optimization of the command and control concepts associated with these systems. Regardless of specific mode (air, surface, or undersea), Navy leaders must fully embrace mission command to fully realize the power of these capabilities.

Unmanned History

“Unmanned” systems are not necessarily new. The U.S. Navy’s long history includes the employment of a variety of such platforms. For example, in 1919, Coast Battleship #4 (formerly USS IOWA (BB-1)) became the first radio-controlled target ship to be used in a fleet exercise.4 During World War II, participation in an early unmanned aircraft program called PROJECT ANVIL ultimately killed Navy Lieutenant Joe Kennedy (John F. Kennedy’s older brother), who was to parachute from his bomb-laden aircraft before it would be guided into a German target by radio-control.5 In 1946, F6F Hellcat fighters were modified for remote operation and employed to collect data during the OPERATION CROSSROADS atomic bomb tests at Bikini.6 These Hellcat “drones” could be controlled by another aircraft acting as the “queen” (flying up to 30 miles away). These drones were even launched from the deck of an aircraft carrier (almost 70 years before the X-47B performed that feat).

However, the Navy’s achievements over the last few years were groundbreaking because the platforms were autonomous (i.e. controlled by machine, not remotely operated by a person). The current discussion of autonomy frequently revolves around the issues of ethics and accountability. Is it ethical to imbue these machines with the authority to use lethal force? If the machine is not under direct human control but rather evaluating for itself, who is responsible for its decisions and actions when faced with dilemmas? Much has been written about these topics, but there is a related and less discussed question: what sort of mindset shift will be required for Navy leaders to employ these systems to their full potential?

Command, Control, and Unmanned Systems

According to Naval Doctrine Publication 6 – Command and Control (NDP 6), “a commander commands by deciding what must be done and exercising leadership to inspire subordinates toward a common goal; he controls by monitoring and influencing the action required to accomplish what must be done.”7 These enduring concepts have new implications in the realm of unmanned systems. For example, while a commander can assign tasks to any subordinate (human or machine), “inspiring subordinates” has varying levels of applicability based on whether his units consist of “remotely piloted” aircraft (where his subordinates are actual human pilots) or autonomous systems (where the “pilot” is an algorithm controlling a machine). “Command” also includes establishing intent, distributing guidance on allocation of roles, responsibilities, and resources, and defining constraints on actions.8 On one hand, this could be straightforward with autonomous systems as this guidance could be translated into a series of rules and parameters that define the mission and rules of engagement. One would simply upload the mission and deploy the vehicle, which would go out and execute, possibly reporting in for updates but mostly operating on its own, solving problems along the way. On the other hand, in the absence of instructions that cover every possibility, an autonomous system is only as good as the internal algorithms that control it. Even as machine learning drastically improves and advanced algorithms are developed from extensive “training data,” an autonomous system may not respond to novel and ambiguous situations with the same judgment as a human. Indeed, one can imagine a catastrophic military counterpart to the 2010 stock market “flash crash,” where high-frequency trading algorithms designed to act in accordance with certain, pre-arranged criteria did not understand context and misread the situation, briefly erasing $1 trillion in market value.9

“Control” includes the conduits and feedback from subordinates to their commander that allow them to determine if events are on track or to adjust instructions as necessary. This is reasonably straightforward for a remotely piloted aircraft with a constant data link between platform and operator, such as the ScanEagle or MQ-8 Fire Scout unmanned aerial systems. However, a fully autonomous system may not be in positive communication. Even if it is ostensibly intended to remain in communication, feedback to the commander could be limited or non-existent due to emissions control (EMCON) posture or a contested electromagnetic (EM) spectrum. 

Mission Command and Unmanned Systems

In recent years, there has been a renewed focus across the Joint Force on the concept of “mission command.” Mission command is defined as “the conduct of military operations through decentralized execution based upon mission-type orders,” and it lends itself well to the employment of autonomous systems.10 Joint doctrine states:

“Mission command is built on subordinate leaders at all echelons who exercise disciplined initiative and act aggressively and independently to accomplish the mission. Mission-type orders focus on the purpose of the operation rather than details of how to perform assigned tasks. Commanders delegate decisions to subordinates wherever possible, which minimizes detailed control and empowers subordinates’ initiative to make decisions based on the commander’s guidance rather than constant communications.”11

Mission command for an autonomous system would require commanders to clearly confer their intent, objectives, constraints, and restraints in succinct instructions, and then rely on the “initiative” of said system. While this decentralized arrangement is more flexible and better suited to deal with ambiguity, it opens the door to unexpected or emergent behavior in the autonomous system. (Then again, emergent behavior is not confined to algorithms, as humans may perform in unexpected ways too.) 

In addition to passing feedback and information up the chain of command to build a shared understanding of the situation, mission command also emphasizes horizontal flow across the echelon between the subordinates. Since it relies on subordinates knowing the intent and mission requirements, mission command is much less vulnerable to disruption than detailed means of command and control.

However, some commanders today do not fully embrace mission command with human subordinates, much less feel comfortable delegating trust to autonomous systems.  They issue explicit instructions to subordinates in a highly-centralized arrangement, where volumes of information flow up and detailed orders flow down the chain of command. This may be acceptable in deliberate situations where time is not a major concern, where procedural compliance is emphasized, or where there can be no ambiguity or margin for error. Examples of unmanned systems suitable to this arrangement include a bomb disposal robot or remotely piloted aircraft that requires constant intervention and re-tasking, possibly for rapid repositioning of the platform for a better look at an emerging situation or better discrimination between friend and foe. However, this detailed control does not “function well when the vertical flow of information is disrupted.”12 Furthermore, when it comes to autonomous systems, such detailed control will undermine much of the purpose of having an autonomous system in the first place.

A fundamental task of the commander is to recognize which situations call for detailed control or mission command and act appropriately. Unfortunately, the experience gained by many commanders over the last decade has introduced a bias towards detailed control, which will hamstring the potential capabilities of autonomous systems if this tendency is not overcome.

Current Practice

The American military has enjoyed major advantages in recent conflicts due to global connectivity and continuous communications. However, this has redefined expectations and higher echelons increasingly rely on detailed control (for manned forces, let alone unmanned ones). Senior commanders (or their staffs) may levy demands to feed a seemingly insatiable thirst for information. This has led to friction between the echelons of command, and in some cases this interaction occurs at the expense of the decision-making capability of the unit in the field. Subordinate staff watch officers may spend more time answering requests for information and “feeding the beast” of higher headquarters than they spend overseeing their own operations.

It is understandable why this situation exists today. The senior commander (with whom responsibility ultimately resides) expects to be kept well-informed. To be fair, in some cases a senior commander located at a fusion center far from the front may have access to multiple streams of information, giving them a better overall view of what is going on than the commander actually on the ground. In other cases, it is today’s 24-hour news cycle and zero tolerance for mistakes that have led senior commanders to succumb to the temptation to second-guess their subordinates and micromanage their units in the field. A compounding factor that may be influencing commanders in today’s interconnected world is “Fear of Missing Out” (FoMO), which is described by psychologists as apprehension or anxiety stemming from the availability of volumes of information about what others are doing (think social media). It leads to a strong, almost compulsive desire to stay continually connected.  13

Whatever the reason, this is not a new phenomenon. Understanding previous episodes when leadership has “tightened the reins” and the subsequent impacts is key to developing a path forward to fully leverage the potential of autonomous systems.

Veering Off Course

The recent shift of preference away from mission command toward detailed control appears to echo the impacts of previous advances in the technology employed for command and control in general. For example, when speaking of his service with the U.S. Asiatic Squadron and the introduction of the telegraph before the turn of the 20th century, Rear Admiral Caspar Goodrich lamented “Before the submarine cable was laid, one was really somebody out there, but afterwards one simply became a damned errand boy at the end of a telegraph wire.”14

Later, the impact of wireless telegraphy proved to be a mixed blessing for commanders at sea. Interestingly, the contrasting points of view clearly described how it would enable micromanagement; the difference in opinion was whether this was good or bad. This was illustrated by two 1908 newspaper articles regarding the introduction of wireless in the Royal Navy. One article extolled its virtues, describing how the First Sea Lord in London could direct all fleet activities “as if they were maneuvering beneath his office windows.”15 The other article described how those same naval officers feared “armchair control… by means of wireless.”16 In century-old text that could be drawn from today’s press, the article quoted a Royal Navy officer:

“The paramount necessity in the next naval war will be rapidity of thought and of execution…The innovation is causing more than a little misgiving among naval officers afloat. So far as it will facilitate the interchange of information and the sending of important news, the erection of the [wireless] station is welcomed, but there is a strong fear that advantage will be taken of it to interfere with the independent action of fleet commanders in the event of war.”

Military historian Martin van Creveld related a more recent lesson of technology-enabled micromanagement from the U.S. Army. This time the technology in question was the helicopter, and its widespread use by multiple echelons of command during Viet Nam drove the shift away from mission command to detailed control:

“A hapless company commander engaged in a firefight on the ground was subjected to direct observation by the battalion commander circling above, who was in turn supervised by the brigade commander circling a thousand or so feet higher up, who in his turn was monitored by the division commander in the next highest chopper, who might even be so unlucky as to have his own performance watched by the Field Force (corps) commander. With each of these commanders asking the men on the ground to tune in his frequency and explain the situation, a heavy demand for information was generated that could and did interfere with the troops’ ability to operate effectively.”17

However, not all historic shifts toward detailed control are due to technology; some are cultural. For example, leadership had encroached so much on the authority of commanders in the days leading up to World War II that Admiral King had to issue a message to the fleet with the subject line “Exercise of Command – Excess of Detail in Orders and Instructions,” where he voiced his concern. He wrote that the:

“almost standard practice – of flag officers and other group commanders to issue orders and instructions in which their subordinates are told how as well as what to do to such an extent and in such detail that the Custom of the service has virtually become the antithesis of that essential element of command – initiative of the subordinate.”18

Admiral King attributed this trend to several cultural reasons, including anxiety of seniors that any mistake of a subordinate be attributed to the senior and thereby jeopardize promotion, activities of staffs infringing on lower echelon functions, and the habit and expectation of detailed instructions from junior and senior alike. He went on to say that they were preparing for war, when there would be neither time nor opportunity for this method of control, and this was conditioning subordinate commanders to rely on explicit guidance and depriving them from learning how to exercise initiative. Now, over 70 years later, as the Navy moves forward with autonomous systems the technology-enabled and culture-driven drift towards detailed control is again becoming an Achilles heel.

Warfare Tactics Instructor: A Unique Opportunity for Junior Officers

Rapid technological advancements and the re-emergence of near peer competition require that we continue to invest in high end tools – platforms, weaponry, and sensors. Equally important are the tactics to employ them and the associated training investment we must make in today’s warfighters and future leaders in the Surface Warfare Officer (SWO) cadre. The centerpiece of an amped-up warfighting culture in surface warfare is the Warfare Tactics Instructor (WTI) program, available to all division officers, department heads eligible for shore duty, and a small number of limited duty and chief warrant officers.

The ideal onramp into the WTI community is during the first shore tour following completion of at-sea division officer assignments. This timing allows the WTI program to fit neatly in a career pipeline. Three attributes set the WTI program apart: the opportunity to develop expertise in areas the Navy needs, exposure to exclusive professional development opportunities during the readiness production tour and throughout a career, and the empowerment to make significant contributions at a very junior level.

Expertise

The ability to develop confidence through professional expertise early in a career has a profound accelerating effect on an officer’s development, and directly contributes to a sense of purpose and fulfillment. WTIs are afforded the time, resources, and experience-building opportunities they need to learn while making substantive contributions to tactics and warfighting proficiency.

The WTI program offers a gateway for young officers to develop deep tactical expertise in the fields of Integrated Air and Missile Defense (IAMD), Anti-Submarine/Surface Warfare (ASW/SUW), and Amphibious Warfare (AMW). Each field begins with a two week Instructor and Tactics Course (ITC) followed by a tailored, 14-16 week course of instruction. During this instruction period, prospective WTIs are mentored and coached to develop their skills at leveraging the Plan, Brief, Execute, and Debrief (PBED) methodology for rapid learning. Following this training, WTIs complete a 24-month “readiness production tour” at SMWDC headquarters or one of four SMWDC Divisions – focused on Sea Combat, IAMD, AMW, or Mine Warfare – or selected training commands (CSG-4/15, TTGP/L, ATG, CSCS, or SWOS, to name a few). During this tour, WTI skills are matured both in the classroom – and at sea – during Surface Warfare Advanced Tactical Training (SWATT) and other fleet training events.

Learning by Teaching

The emphasis on teaching as a basis for learning is based on an idea espoused by the Roman philosopher Seneca, who declared, “docendo discimus” or, “by teaching, we learn.” This model of learning is also used to develop WTI candidates, which is why instructor skills are a main focus of ITC. Quality of lesson delivery is established through a rigorous standardization process that must be completed for each lecture delivered by a WTI. It’s not uncommon for a WTI to invest weeks or months of research, as well as conduct numerous “murder boards” with fellow WTIs, technical experts, and senior officers, before presenting at the podium. The process is meant to maintain a high standard of instruction where WTIs have established mastery of content and exhibit confidence in delivery.

Focused Specialty Areas

During initial WTI training, students are assigned relevant tactical projects that match critical fleet needs and account for student interests. Projects often involve new technology or capability that must be thoughtfully and effectively integrated into maritime warfare doctrine. Other projects center on updating existing doctrine or repurposing existing systems in new and innovative ways. Specialty areas and projects are assigned based on WTI preference and crosscut broadly, from high-end tactics to training systems and learning science.

Focus area research often extends past initial WTI training, into subsequent readiness production tours, and beyond. SMWDC provides mentorship, applies resources, and opens doors to connect WTIs to thought leaders, technical community experts, industry partners, and community leaders to develop their specialty area work.

Coaching and Training Skills

WTIs are the core workforce of SMWDC’s advanced tactical training at sea. They rely on replay tools that include systems data, voice, and other information to rapidly build ground truth and facilitate debrief sessions. Equipped with irrefutable data on what really happened, the “I thought” and “I felt” ambiguities are driven out of the debrief process, enabling shipboard watch teams to learn and grow together more rapidly.

The combination of WTI knowledge, replay-assisted PBED, and specialized training focused on team dynamics and coaching skills offers a powerful method for improving learning across the fleet. The aim is to create an environment of transparency and mutual trust among watch team members, where Sailors enter debrief sessions eager to identify their own shortfalls in order to improve team and unit performance.

At-sea training allows WTIs to observe multiple ships and teams across a variety of training and operational circumstances. The WTIs gain practical insight into how doctrine plays out on the deckplates, as well as hone their ability to identify team performance issues during at-sea training. While the immediate objective is to improve tactical proficiency and unit performance, the skills WTIs gain are extraordinarily useful in future roles as department heads.

Performance Analysis

The final link in WTI expertise development leverages the strong partnership between SMWDC and the technical community. Our ability to measure and analyze performance among units is a challenge due to complex weapons systems, ship configuration variance, and the number of watchstanders distributed in different controlling stations. To build a clear picture of how tactics, training, and systems converge into warfighting capability, a detailed event reconstruction must take place that considers system actions, operator actions, and tactics.

Naval Surface Warfare Center (NSWC) Corona, Naval Undersea Warfare Center (NUWC) Keyport, and SMWDC have developed a Data Analysis Working Group (DAWG) to conduct performance analysis of SMWDC training events. The intent is to extract empirical, data-driven insights from the careful analysis of systems, operators, and tactical performance.

The process is laborious, but straightforward. Following at-sea training, event data is extracted from unit combat systems and sensors and then brought to NSWC for detailed analysis. Following initial analysis from the technical community, WTIs and SMWDC leaders stand up a 1-2 week DAWG event.

By examining system performance, operator performance, and tactics as a consolidated effort, the process can lead to discoveries not captured by direct observation – system anomalies, operator actions, and flaws in tactics. Findings and lessons learned can be very useful because they are underpinned by empirical data and technical analysis. To date, more than 40 weapons system performance anomaly reports have been generated from DAWG events. Systems issues have been identified and funneled to the appropriate technical community to resolve, tactics have been updated, and numerous operator performance issues have been provided to the training community as opportunities to grow or strengthen curriculum. This allows SMWDC to advocate for tactical updates among partner warfighting development centers and provide feedback to the TYCOM and Surface Warfare training enterprise.

For the WTI, immersion in performance analysis activity with civilian technical experts offers a unique lens into how weapons systems, operator performance, and tactics are all linked to create combat potential.

Professional Development

Because the program is highly sought after by driven, focused professionals, the majority of WTIs are on track to return to sea as department heads. Notably, WTI cadre retention is double historical averages in the Surface Warfare community at roughly 70 percent. WTIs heading back to sea have a notable advantage given the training they receive and the experiences they gain at a formative stage of their career that others simply do not.

Assignment Consideration

Similar to officers with other subspecialty skills – Nuclear Program, Financial Management, Operations Analysis, and Space Systems – WTIs have unique skillsets based on their focus areas. For example, IAMD WTIs in readiness production tour billets at the Naval Air Warfare Development Center in Fallon, Nev., have completed the Carrier Airborne Early Warning Weapons School, becoming dual-patched WTIs. These officers are among very few in the Navy with expertise in Integrated Fire Control (IFC) from both the Aviation and Surface perspectives.

To maximize the return on investment for these unique WTI skills, SMWDC is closely aligned with PERS-41 in the distribution process, ensuring future assignments leverage these strengths (e.g., assigning a WTI with IFC expertise to IFC-capable units). While assignments will always consider many variables, this close relationship ensures WTI experience and skills are considered during the assignments process. 

Continuing Education

WTI training and readiness production tours leave less time to complete graduate education between division officer and department head assignments. To mitigate this challenge, WTIs are awarded priority for graduate degree programs at service colleges as well as the Naval Postgraduate School distance learning programs.

Additionally, WTIs are afforded unique and exclusive professional development opportunities that extend throughout their careers. Annual “Re-Blue” events held at SMWDC Divisions are a venue for WTIs, both in-and-out of readiness production tours to attend week-long immersive workshops where information is exchanged and re-distributed into the fleet. Funded travel to Re-Blue events keeps WTIs connected to the sharp edge of the operational Fleet during their readiness production tours and beyond. Re-Blue events are an example of SMWDC’s commitment to maintaining excellence within the WTI cadre.

Empowerment

SMWDC is unlocking the potential of our junior officers and post-department heads, empowering them to swarm and solve difficult problems. While experience will always have a place at the table, this new generation of naval officers holds several key advantages. Unencumbered by “the way things have always been,” these officers are better suited to envision a future that leverages trends in technology, communication, and learning. This is an area where fresh perspective is an asymmetric advantage. WTIs bring their creativity, ingenuity, and initiative to developing the next generation of cutting-edge tactics, techniques, and procedures.

WTI’s are creating a positive impact in the Fleet. From immersion in their focused specialty areas to tactical projects, and deckplate innovations, WTIs have built an impressive list of contributions since SMWDC’s formal establishment in June 2015. Consider the below examples of projects inspired, developed, and built by WTIs, while being supported by SMWDC leadership.

  • Lt. Cmdr. Katie Whitman was the lead action officer developing the SWATT in port and underway curriculum from the ground-up, using best-of-breed practices culled from aviation and other communities. She developed replay-assisted PBED for rapid learning and crafted the SWATT performance analysis strategy, which are now distinctive features of the exercise.
  • Lt. Ben Graybosch partnered with NUWC Keyport to revise the VISTA replay tool to include A/V-15 sonar system data, enabling the detailed “ground truth” ASW replay for unit sonar teams within 4 hours of completing ASW events. Graybosch’s effort moved the needle on ASW ground truth replay availability from days or weeks down to hours after an event. With replay tools that offer ground truth much earlier, we can increase the velocity of learning within surface ASW teams dramatically. VISTA is now employed in every ASW event supported by SMWDC and other fleet training events.
  • Lt. Brandon Naddel was the lead author for the Naval Surface Gunnery Publication released in 2017. Naddel and his team revised a 15-year-old document laden with technical jargon and dated systems into an information-packed and easily understood tactical publication relevant to all surface ships.
  • Lt. Tyson Eberhardt authored tactical guidance for the emerging Continuous Active Sonar (CAS) capability. Eberhardt leveraged at-sea training and experimentation events to rapidly refine tactical guidance in 2017. Based on his work, the CAS capability was used to great success in the operational fleet later that year.
  • Lt. Matt Clark designed and built a Target Motion Analysis (TMA) training tool accessible on any classified terminal with built-in performance analytics. Clark’s tool has potential to provide insight on the rate of individual skills decay in TMA. This type of information could then be used to inform currency thresholds for future training requirements.
  • Lt. Aaron Jochimsen was the lead author for the SM-6 TACMEMO. He conducted extensive research on SM-6 that included production site visits, participation in wargaming and experimentation, as well as involvement in fleet missile firings.
  • Chief Warrant Officer Troy Woods completed a readiness production tour with the Center for Surface Combat Systems, where he was involved in training individuals and teams on IAMD skills. Woods was subsequently assigned to USS BUNKER HILL (CG 52), where his skills are being put to use as lead IAMD planner within the Theodore Roosevelt Carrier Strike Group. Woods attended the IAMD WTI Re-Blue event in Dahlgren, Va., to share the operational perspective with his fellow IAMD WTIs and receive the latest tactical information from SMWDC IAMD Division leadership.

The WTI Program is a career opportunity that values our officers and empowers them to solve complex and challenging problems. SMWDC WTIs naturally have an eye toward innovation, are re-building the surface warfare library of tactical guidance, are shepherding new capability from delivery to operational success, and challenging the status quo in surface warfare training. Lt. Jochimsen, the lead author of the SM-6 TACMEMO, said it best:

“The opportunity to develop deep knowledge – Subject Matter Expertise – is a game-changing confidence builder as a junior officer. I feel much more prepared for the challenges of an at-sea department head assignment after completing a WTI readiness production tour.”

Conclusion 

The WTI cadre of warriors, thinkers, and teachers are uniquely equipped with the experience and knowledge to make significant contributions during their readiness production tours and throughout their careers. It is no coincidence that the same skills involved in developing tactical mastery are extraordinarily useful in subsequent assignments at sea – department head, XO, CO, and major command.

While statistically significant trend data does not yet exist for WTI selection for career milestone billets, members of the WTI cadre performed very well during recent administrative boards.

General Quarters: Evolving Combat Casualty Care at Sea

Medicine is a continuously evolving field, constantly learning from previous experience and improving. This is all the more true in the wartime trauma environment where resources are limited, conditions are austere, and time is either too short or too long. Our brothers and sisters ashore learned through Viet Nam and the early days of Iraq and Afghanistan that combat injuries will become combat fatalities unless personnel on the scene can stabilize the wounded for treatment by a higher echelon of care. As we consider a return to great power conflict and war at sea, our maritime forces should avail themselves of these lessons in order to prevent unnecessary losses of life in future combat.

A Revolution in Combat First Aid

Some wounds are almost always unrecoverable – penetrating head traumas, catastrophic injury to the thoracic cavity, or incapacitation of the central nervous system for instance. However, 20th century conflicts demonstrated that there are significant numbers of preventable battlefield deaths caused by two easily stabilized conditions: bleeding out (exsanguination) and sucking chest wounds (tension pneumothorax). Within one oft-cited research category (infantry casualties in Viet Nam), nearly 60 percent of preventable casualties were from exsanguination due to extremity bleeding and about 33 percent from tension pneumothorax.1

Fast-forward to the mid-1990s when special operation forces (SOF) medical providers began implementing a program known as “Tactical Combat Casualty Care” (TCCC). Their objective was to increase survivability amongst SOF elements by improving the trauma intervention capability and equipment of several, if not all, team members. Specifically, a greater emphasis was placed on controlling bleeding through properly employed tourniquets or hemostatic agents and alleviation of tension pneumothorax through needle decompression. As OIF and OEF repeatedly validated the effectiveness of TCCC in the SOF community, the training proliferated to conventional forces, becoming a cornerstone of modern deployment readiness. Consequently (and alongside other field medicine advances), the rate of service members being killed in action or ultimately dying of wounds is a fraction of previous conflicts.

Combat at sea will be somewhat different. For instance, penetrating trauma from discrete projectiles (e.g., bullets) will not be as prevalent as in land warfare, but similar wounds resulting from shrapnel or fragmentation will likely be common. Numerous additional and relevant mechanisms of injury are also possible during a surface combat scenario. Consider detonation of an anti-ship cruise missile close aboard or within the skin of the ship resulting in primary and secondary blast injuries, burns, blunt force trauma, as well as neurologic injuries without other outward signs of injury – all conditions similar to those seen aboard USS Cole.2 Individual crewmembers’ ability to intervene and stabilize some of these cases will no doubt improve survival rates as well as assist in maintaining combat effectiveness.

Behind the Times

While large deck warships (e.g. CVN, LHD) deploy with embarked top-of-the-line medical teams, the majority of the U.S. fleet does not. Cruisers and destroyers routinely put to sea with a well-trained independent duty corpsman (IDC, aka “Doc”) as the primary provider assisted by 1-2 junior corpsmen and a cadre of stretcher-bearers. LCS medical manning is even more constrained, usually a single IDC aided by a smaller number of stretcher-bearers. This has been an adequate arrangement for steady state surface operations (and the inspection-centric training cycles) of the past 30 years, but will not be sufficient if the fleet finds itself once again in a shooting war.

Outside of in-rate training for Corpsmen­ – which has already incorporated trauma lessons from Iraq, Afghanistan, etc.– first aid training aboard surface vessels has yet to advance much beyond U.S. civilian standards of care (basic first aid, CPR, and BLS). These standards do not account for the priority of combat operations over medical treatment, the increased lethality of combat and the shipboard environment, or extended timelines from point of injury to definitive care facilities (factors that led to TCCC’s inception). According to one Chief with extensive expeditionary experience and who has facilitated antiterrorism/ force protection (AT/FP) assessments of East Coast warships over the past three years, “Concepts of direct pressure, pressure points, and proper use of tourniquets are just not a thing out there.” Another Chief was told separately that pressure point manipulation for arterial bleeding was too advanced for shipboard use.

Surface force equipage has seen some TCCC-based upgrades in the past few years – better chest seals, hemostatic agents, and cricothyrotomy kits were improvements cited by a current IDC. However, mass casualty stations and first aid boxes are often still filled with antiquated equipment: less effective elastic tourniquets, basic gauze, medical tape, etc. While still helpful, there are better and more efficient tools available in the joint inventory. Additionally, if one were to ask a first tour junior officer or Sailor about an Individual First Aid Kit (IFAK), one might well receive a description of the small boxes commonly found at commercial retailers rather than a vital piece of military kit. These life saving pouches are not a standard-issue item and are often only present aboard vessels whose 1) whose Doc has encountered them (and their importance) elsewhere, and 2) whose discretionary budgets have permitted some quantity of acquisition (e.g. for armed watch standers).

What is most beneficial to the surface force, however, is the fact that casualty management is already practiced as part of general quarters, main space fire, or dedicated mass casualty drills. Rest assured, medical providers and corpsmen are thinking about these matters even when the rest of us are not. Additionally, Doc already has that cadre of stretcher-bearers who have been given an introduction to treating injuries, and there is already a system of pre-staged equipment outside of sickbay. These factors provide a ready-made infrastructure for the surface force to improve on.

TCCC for Tomorrow

The current medical manning construct aboard small deck warships has been adequate for non-combat operations in the post-Cold War era. The force is well-positioned to stabilize the occasional industrial traumas that can occur aboard vessels, or to manage larger groups of minor injuries. Since the 1990s, true tests of our readiness to simultaneously manage trauma and combat have been blessedly few: Cole in 2000 and Firebolt in 2004. Even then, both events were instantaneous and permitted crews to address medical emergencies without having to continue combat operations. That is an unlikely luxury during a strait transit actively contested by small boat swarms, or an open ocean patrol under enemy missile, torpedo, or gun attacks. 

The most fundamental change brought by TCCC was the universal carriage of an IFAK along with the training to use its contents. The kit itself contains the basic material to control traumatic bleeding, decompress a tension pneumothorax, and otherwise stabilize the service member until better care arrives or they are medically evacuated. The training aspect of this cannot be emphasized enough: classroom introduction to principles and equipment followed by periodic drills under varying levels of stress. Every Sailor afloat should don an IFAK with their general quarters kit, and be practiced on its use under stressful circumstances – i.e., loud, difficult, and strenuous– as part of combat drills.

In addition to IFAKs and training for the entire crew, the fleet needs to upgrade stretcher-bearer training from basic first aid to contemporary TCCC standards. Numerous curricula are already available to the fleet as well as training aids that have been developed through years of preparing ground troops. Similarly, mass casualty boxes and aid stations should be standardized with contents that will enable these TCCC-trained stretcher-bearers to implement the training they received. In deference to the different wound mechanisms likely to be seen in maritime combat, burn care supplies (e.g., Waterjel) should figure prominently in these kits.

Conclusion

What if the missiles fired in the Red Sea at Mason, Nitze, and Ponce had found their mark? Those reported Silkworm variants carry 300-500kg warheads. Would we instead be discussing a case study in TCCC rather than a recommendation? A smaller successor, the C-802 (160-300kg warhead), had recently struck the Swift while in service with the Emirati Navy and although casualty reports were minimal to none, photos of the damage (note the destroyed pilot house) make that assertion unlikely. It took collisions aboard Fitzgerald and McCain to give traction to problems long known to surface warriors and re-order some priorities. We do not need to suffer another such tragedy in order to update our ability to manage combat trauma at sea.

Time will always be in short supply, thus prioritization is paramount in preparing crews to go to war. If we are returning to an emphasis on maritime warfighting, then we must be competent at more than just navigation and engineering. Like damage control, TCCC and other individual combat skills should be regarded as fundamental aspects of modern naval service – one more way in which we equip the man (and woman) rather than just man the equipment. The training, tools, and resources for TCCC are already available through the Navy’s medical and logistics systems, the surface force need only take heed of it.

The Gate of Tears: Interests, Options, and Strategy in the Bab-el-Mandeb Strait

Introduction

Some were surprised by the news that four U.S. soldiers were killed in an ambush in Niger and may have even wondered “why do we have troops in Africa?” Some also agree with the recent bipartisan call from Congress to end U.S. military support to the Saudi Arabian-led coalition’s intervention in Yemen. In this context, the Strait of Bab-el-Mandeb, the narrow waterway between the Arabian Peninsula and northeast Africa, might sound inconsequential, but is in fact critical to U.S. national security interests.

Arabic for “the Gate of Tears,” the strait could not be more appropriately named. Situated at the center of the some of the world’s most critical humanitarian disasters and economic issues, it is a decisive point for U.S. foreign policy in the Middle East and the global economy more broadly. To the north lies Yemen, a failed state where 17 million people face starvation, over a million people are stricken with cholera while thousands have died from the disease, a diphtheria outbreak has emerged, and multiple regional actors and terrorist networks vie for power in a bloody conflict that has claimed over 5,000 civilian lives in three years.

To the south lies Africa where fragile states like Somalia battle famine and Islamic militant groups. Atrocities abound at sea as well. Refugees cross the Bab-el-Mandeb to the north and south because both sides seem to be the better alternative. In March 2017, 42 refugees fleeing from Yemen to Sudan were shot and killed by an attack helicopter, which apparently mistook them for militants.1 In August 2017, smugglers threw over 300 Somali refugees into the water near the Yemeni coast, drowning up to 70, in the span of 24 hours after reportedly sighting authority figures on shore.Just last week, 30 African refugees drowned en route from Aden to Djibouti, after their boat capsized amid gunfire from smugglers.

Meanwhile, the Bab-el-Mandeb serves as a critical artery for the global economy. 52 vessels and 4 million barrels of oil transit the strait per day, making it the fourth busiest waterway in the world, and the only one surrounded by chaos. In the last year, two merchant vessels and five naval ships have been attacked with cruise missiles, explosive boats, and small arms in the southern Red Sea, while the very real threat of mines exists lurking in the water.

Yemeni and Somali civil wars, humanitarian disaster, and the economic importance of the Bab-el-Mandeb form a complex dynamic in which to develop U.S. foreign policy. The key for policymakers is to determine what U.S. national interests are at stake and what actions must be taken to protect them. After careful consideration of the issues surrounding the Bab-el-Mandeb, it becomes clear that the U.S. can ill afford to do nothing. On the other hand, it may also be unaffordable to secure U.S. interests by leading multilateral stabilization efforts via intervention within Yemen and Somalia. Fortunately, a third alternative is available via American seapower. Applying maritime influence to enable other elements of national power can contain threats to national security and the global economy, while providing a path to mitigate human suffering in the long term.

U.S. National Interests and the Strategic Calculus

The debate over U.S. national interests will not be settled here, but for simplicity’s sake and the purpose of meaningful analysis, the following definitions from the July 2000 Commission on America’s National Interests will be used:3

  • Vital – Vital national interests are conditions that are strictly necessary to safeguard and enhance Americans’ survival and well-being in a free and secure nation. A key example is to “ensure the viability and stability of major global systems (trade, financial markets, supplies of energy, and the environment).”
  • Extremely Important – Extremely important national interests are conditions that, if compromised, would severely prejudice but not strictly imperil the ability of the U.S. government to safeguard and enhance the well-being of Americans in a free and secure nation. A key example is to “Prevent the emergence of a regional hegemon in important regions, especially the Persian Gulf.”
  • Important – Important national interests are conditions that, if compromised, would have major negative consequences for the ability of the U.S. government to safeguard and enhance the well-being of Americans in a free and secure nation. A key example is to “discourage massive human rights violations in foreign countries.”

Due to the importance of the Bab-el-Mandeb Strait to the global and, consequently, U.S. economies, the U.S. has a vital national interest in maintaining the free flow of commerce through the Strait. Even if the U.S. did not depend on the 1.5 billion barrels of oil (currently $98B) annually that flow through the Strait, allies in Europe would certainly feel the economic impact if shipping companies re-routed their vessels around the Cape of Good Hope in the event of crisis or conflict. Inflated maritime insurance rates and an additional 10 days transit time from the Middle East to the U.S. would have considerable worldwide ripple effects to which the U.S. economy would not be immune. Moreover, as one of two primary sea lines of communication to the CENTCOM AOR, the U.S. also has a military interest in maintaining freedom of navigation through the Bab-el-Mandeb Strait. In October 2016, the U.S. was forced to respond with Tomahawk missile strikes into western Yemen when the USS Mason came under anti-ship missile fire from the Yemeni coast. Further attacks would increase risk and necessitate additional escorts for Bab-el-Mandeb Strait transits.

Conversely, the U.S. has no vital national interest in broader involvement in the armed conflicts in Yemen and Somalia; however, its interests are clearly impacted by the growing threat emanating from Yemen. In western Yemen, Saudi Arabia along with eight other regional allies, are fighting a brutal war against separatist Houthi rebels who aim to establish an anti-Western Shia government in Yemen. In December 2017, U.S. Ambassador Nikki Haley provided material evidence to the international community that Iran provides missiles and advanced weaponry to the Houthis, enabling them to target vessels transiting the Bab-el-Mandeb Strait and strategic locations inside Saudi Arabia, threatening the U.S. national interest in free flow of commerce and advancing its own interest in promoting its regional hegemony.4,5 In January 2018, the Houthis threatened to close the Red Sea to international shipping if the Saudi-led Coalition continued its advance toward Hudaydah, a strategically important Houthi-held port critical to the flow of humanitarian aid. If the Houthi threat is credible, Iranian-aligned forces could now threaten another vital maritime chokepoint, in addition to the Strait of Hormuz which Iran has often threatened to close. Growing Houthi influence and capabilities in Yemen also impose costs and shift the balance of power in relation to Iran’s regional opponents, primarily Saudi Arabia, and allows Tehran to expand its influence elsewhere. Saudi Arabia may have felt compelled to intervene in Yemen’s civil war as the tide shifted in favor of the Houthis because the prospect of bordering an Iranian-aligned state would prove strategically disadvantageous.

In eastern Yemen, Al Qaeda in the Arabian Peninsula (AQAP) and ISIS-Yemen maintain presence, despite UAE-led counterterrorism operations, due to lack of effective governance and internal security. Despite pressure from the West, AQAP remains a threat to the U.S. homeland, and the prominence of ISIS-Yemen continues to grow as the extremist caliphate is gradually eliminated in Iraq and Syria. Without a doubt, the U.S. has a vital national interest in supporting its Gulf Cooperation Council (GCC) partners in their counterterrorism efforts and in defense of their borders, but it does not benefit from becoming directly entangled in the fight.

Similarly, Somalia provides a safe haven for terrorists who would do harm to U.S. interests at home and abroad. Fighting rages on between the Federal Government of Somalia, which was only recently established after two decades of near-anarchy, and the Al-Qaeda aligned militant group Al Shabaab, while the civilian population suffers the consequences. Meanwhile, criminals continue to recruit disenfranchised young men, desperate and angry with perceived (and sometimes real) illegal fishing in Somali territorial waters, to become pirates. Still, even though Somalia is a hallmark of instability in the region and a safe haven for terrorist organizations, the U.S. has no vital national interest in establishing security and governance in the African country. From Vietnam, to Somalia itself in the 1990s, America has learned through experience the high cost of entering into regional and internal armed conflicts in proxy pursuit of national interests. Becoming directly entangled in the conflicts surrounding the Bab-el-Mandeb is counter to U.S. national interests, but so is ignoring them. U.S. vital national interests in Somalia and in Yemen are limited to ensuring instability is contained within territorial borders so that the free flow of commerce is maintained, attacks against the homeland and assets abroad are prevented, and American influence in the region is sustained.

In Pursuit of National Interests

Considering U.S. vital national interests in the Bab-el-Mandeb and surrounding territories – maintaining the free flow of commerce, preventing attacks against the homeland and assets abroad, and sustaining American and allied influence in the region – the challenge arises when deciding how to apply the instruments of national power (diplomatic, informational, military, and economic) to secure those interests. The U.S. could secure its interests by:

1) Leading a large-scale multilateral stabilization effort for Yemen and Somalia

2) Containing threats to national interests via maritime influence; or

3) Taking no action (Isolationism)

Option 1: U.S.-led Stabilization Effort

Bringing all instruments of national power to bear on the Bab-el-Mandeb Strait by leading multilateral stabilization efforts in Yemen and Somalia is a thorough, long-term approach to securing U.S. national interests; however, it would come at significant cost. Securing national interests via stabilization could involve, in varying degrees: state building, military intervention, occupation, and inevitably comparisons to “neo-imperialism” and “adventurism.” In theory, these activities could give rise to effective governance, which would serve to eliminate the threats to U.S. national interests spilling out of Yemeni and Somali borders. The governments, in this case the Federal Government of Somalia and Republic of Yemen Government, ideally would be able to aid in securing the Bab-el-Mandeb Strait, stamp out terrorist safe havens, and provide adequate food and medical care to their populations, all while acting in alignment with U.S. foreign policy objectives.            

The trouble is that stabilization has rarely turned out the way the U.S. intended. The most striking examples are Iraq and Afghanistan, where the U.S. remains after intervening to eliminate threats to the homeland and the region. Supporting Contra rebels in Nicaragua in the 1980s or the present-day Syrian Opposition are two examples where the U.S. applied pressure indirectly with less than desirable results. Meanwhile, it’s been 25 years since the U.S. first led a UN coalition into Somalia, tasked with stabilizing the war-torn country whose central government had collapsed. Clearly, stabilization efforts are no sure thing. Even when stabilization is successful, the cost is immense. Following World War II, in addition to the over $14B ($140B in today’s dollars) given in aid to Germany and Japan for economic recovery, the U.S. maintained a significant military presence to help stabilize those countries.6,7 It is reasonable to assume that a concerted effort to stabilize Yemen and Somalia would lead to indefinite military presence in those countries as well. Fortunately, there is an alternative to stabilization for securing U.S. national interests in the Bab-el-Mandeb region: maritime influence.              

Option 2: Containing Threats to Yemen and Somalia – Maritime Influence

Rather than directly stabilizing Yemen and Somalia, the U.S. can ensure the free flow of commerce and secure its foreign policy objectives in the region by exerting maritime influence and projecting its power landward. Through a combination of naval operations, international cooperation, and engagement with industry, the U.S. can mitigate the risk to commercial and friendly naval vessels transiting the Bab-el-Mandeb Strait, albeit not without the presence of forces as a credible deterrent to would-be attackers. Securing the critical waterway can help secure U.S. foreign policy objectives in the region by eliminating maritime attacks as an option for belligerents in internal conflicts and forcing their attention inward, but airstrikes or other direct action may be required to drive home the point that the Bab-el-Mandeb is “out of bounds” and the consequences for attacking neutral parties are severe. Ultimately U.S. maritime influence could contribute to international pressure to peacefully end the Yemeni Civil War, and fortify the fragile Somali government. Meanwhile, maritime assets could act as seabases for forces providing humanitarian aid or conducting raids on terrorist networks. The agility the Navy provides – in the form of hospital ships, aircraft carriers, amphibious ships, various surface combatants and patrol craft, sealift and logistics ships, landing craft, helicopters, and other aircraft – has been on display in disaster relief efforts such as the Indonesia tsunami in 2004 and the Haiti earthquake in 2010. These same assets can be used to support counterterrorism efforts as expeditionary mobile bases, allowing special forces to conduct short-duration operations with minimal footprint on land. Lastly, interdiction of lethal aid flowing into Yemen, enabled by UN Security Council Resolutions, would be a key element of maritime influence.

International maritime operations are already underway in the region that the U.S. could leverage to build a maritime influence strategy. U.S., allied, and other international navies, such as China, Russia, and Iran, maintain presence in the Gulf of Aden. These navies already work with organizations such as the U.S. National Geospatial-Intelligence Agency (NGA) and U.K. Maritime Trade Operations (UKMTO) to advise industry of potential threats to shipping. The key to success of a maritime influence strategy will be cooperation of these international navies, especially since the U.S. cannot afford to take on this security burden alone. The U.S.-led Combined Maritime Forces (CMF) offers a viable model for bringing to together naval forces of 32 countries, some of whom would not normally form military partnerships with each other, to provide maritime security in the Middle East. Mutual participation of both Saudi Arabia and Iran, although seemingly unlikely, should be a goal for U.S. policymakers, as this could set the stage for dialogue to mitigate the humanitarian crisis in Yemen. Additionally, contrary to conventional U.S. foreign policy, U.S. leaders should consider recognizing Iran for their contribution to international counter-piracy efforts off Somalia, while holding Saudi Arabia accountable for their role in contributing to the devastating humanitarian conditions in Yemen. 

The international campaign to combat the Somalia piracy epidemic in the early 2000s provides an ideal example of how maritime influence has been proven to effectively eliminate threats at sea. From 2005 to 2012, pirates attacked nearly 700 ships in the Somali basin, and collected $400M in ransom payments. Over that time, the international community gradually came together to address the problem. The U.S. established a multinational maritime coalition to counter piracy threats, and participated in EU and NATO task forces as well. As many as 20 international warships patrolled the waters around the Horn of Africa at any given time. Simultaneously, the U.S. coordinated with the International Maritime Organization (IMO) to establish a set of industry best practices, the latest of which, Best Management Practices, Rev. 4 (BMP4), officially endorsed unarmed embarked security teams.[8] Other maritime influence efforts included establishing an internationally recognized transit corridor, real-time vessel tracking and communication with maritime agencies, and development of mechanisms for transfer of detained pirates to cooperative local governments. This international effort to push piracy back to land was effective – pirates did not successfully hijack a single vessel in the Somali Basin from 2013 to 2016. It is important to note, however, that piracy will remain a threat until it becomes cost prohibitive. Criminals still take advantage of poverty and conflict in Somalia to recruit disenfranchised young men. In 2017, a series of six attacks raised concerns that the piracy epidemic had returned. Instead, it turned out that industry had become lax in applying best practices and about half as many warships patrol the area as in 2012. Not coincidentally, NATO concluded its counter-piracy operation in December 2016. Pirates seemingly sensed an opportunity. Fortunately, no ransoms were paid and the international community once again applied maritime influence to beat back the piracy threat.

Maritime influence was effective in pushing piracy back to shore, and it essentially removed piracy as an option for financial gain in Somalia. In fact, judging from the improvements in Somalia over the last decade, from growing GDP and livestock exports and a democratic presidential election in 2017, one could argue that maritime influence contributed to better conditions on land. Still, the root conditions in Somalia that led to the problem in the first place persist. Maritime influence is not a foreign policy panacea. In the Bab-el-Mandeb region, the U.S. would need to apply maritime influence while supporting international stabilization efforts to make meaningful progress toward resolving the humanitarian crises in Yemen and Somalia, eliminating terrorist safe havens, and effectively securing its national interests. Thus, the U.S. still risks becoming entangled in foreign wars. The U.S. needs to consider whether investing in the security of Yemen, Somalia, and the Bab-el-Mandeb is worth the cost. Of course, the third option is to invest nothing and accept the potential consequences.

Option 3: Isolationism

It can be tempting to assume the U.S. should do nothing at all to stabilize the region around the Bab-el-Mandeb, especially amid the rising tide of nationalism and isolationism in America. After all, many argue that the U.S. should withdraw from the Middle East completely. The danger is that the humanitarian crises in Yemen and Somalia may reach the point of catastrophe, at which the U.S. could be compelled to act purely on the basis of respect for humanity. On a grand enough scale, the alleviation of human suffering, much like the prevention of genocide, can in fact be a vital national interest. There are already 20 million people starving and over one million people suffering from cholera and diphtheria in the two countries surrounding the Bab-el-Mandeb, an unparalleled concentration of human suffering. A particularly heavy rainy season, or even a single cyclone, could rapidly exacerbate shortages of food and medical care. At some point, the loss of life could become so immense that the U.S. has no choice but to intervene, regardless of its security or economic interests. Such an intervention would inevitably come at great cost in terms of dollars and foreign policy objectives, not to mention the risk to American armed forces and civilians on the ground.

Another consequence of doing nothing to stabilize the region is the power vacuum that will likely continue to grow. In Yemen, AQAP and ISIS will unquestionably continue to plot and orchestrate attacks against U.S. interests, barring a concerted U.S. or international effort. In Somalia, the U.S. and other allies provide military support to the government’s fight against Al Shabaab and other militant groups. Without American involvement, both countries would be safe havens for those who would do Americans harm. Further, wherever the U.S. has withdrawn presence and influence throughout the Middle East, states such as Iran, Russia, and China predictably increased their operations in those areas. In August 2017, China opened its very first overseas naval base just south of the Bab-el-Mandeb Strait in Djibouti, signifying the strategic importance China places in the region. With American withdrawal, the U.S. and global economies would be dependent on foreign efforts to secure the Bab-el-Mandeb Strait, whatever those may be. In any case, increasing maritime threats may inevitably force the U.S. foreign policy hand, only the response would be reactive instead of proactive, dictated by the enemy’s actions.

Conclusion

The U.S. can afford neither to ignore the threats emerging from the Bab-el-Mandeb Strait and surrounding territory, nor can it afford to aggressively intervene in Yemen and Somalia wholesale to fully stabilize the region. The most affordable approach to securing U.S. interests in the region is through maritime influence to enable regional and international partner efforts. By leading an international naval, diplomatic, and economic campaign, augmented with key activities internal to Yemen and Somalia, the U.S. can ensure the free flow of commerce, prevent attacks on American citizens, and preserve its influence in the region, while setting the stage for resolution of internal conflicts and humanitarian crises. It may seem heartless to take such a calculated approach to secure one’s own interests in the face of so many others’ suffering. It is helpful, however, to consider that the best way to mitigate that suffering and secure U.S. national interests may be one and the same. Under the wholesale intervention and stabilization approach, humanitarian conditions will not improve at all unless the U.S. is willing and able to adequately resource an international effort, which seems unlikely. Stabilization of Yemen and Somalia through U.S. intervention is simply not a feasible option. The U.S. may not be able to keep the Bab-el-Mandeb Strait from living up to its Arabic name, “the Gate of Tears,” but the best approach to securing its national interests, by exercising maritime influence, also happens to represent the best opportunity to positively impact humanitarian and security conditions in the long run without risking excessive entanglement.
